Registration for the 2012/2013 school year will begin on March 5, 2012 for families new to Notre Dame School.

  • 2011 - 2012 Tuition Rates for preschool through 8th grade
  • New student Registration Form for preschool through 8th grade
  • Tuition Contract for preschool through 8th grades. Make registration fee checks payable to Notre Dame School.
  • Parish Affiliation Form for preschool through 8th grade. Notre Dame Parish members should complete form to Pastors signature and then return form to the school office. The school office will verify your status. Other Parish members must have their Pastor’s signature on the form before you return it to the school office.
  • Child Information form for preschool, pre-kindgarten and kindergarten students
  • Extended Care form for studetns in kindergarten through 8th grade who will need after school care
  • Physical form for preschool, pre-kindgarten and kindergarten students

Assurance Statement of Compliance With the Purpose of Title IX Education Act -
The elementary and secondary Catholic schools of the Archdiocese of Denver, under the jurisdiction of the Most Reverend Charles J. Chaput, O.F.M.
